Care Coordinator – Permanent Part-Time

An exciting opportunity exists for a qualified Registered Nurse to join the team as a Care Coordinator at St Joseph’s Nursing Home.

St Josephs Nursing Home, Lismore is a 130 bed Accredited Aged Care Facility owned and operated by the Trustees of the Roman Catholic Church, Diocese of Lismore since 1936. St Josephs and Mary’s Grange are located on the grounds of St Vincent’s Private Hospital . The facilities specialise in high care services for the frail aged, palliative care and dementia residents. St Joseph’s Nursing Home aim  to provide quality aged care where the relationships between the residents, relatives and staff are based on mutual respect, honesty, openness and shared commitments  The facility enjoys a reputation for high quality care. St Joseph’s is seeking to fill the following permanent  part-time position (72 hours per fortnight):

Reporting directly to the Manager Aged Care – St Joseph’s Nursing Home, the Care Coordinator will work in partnership with our residents and their families in setting resident-centred goals and priorities, care planning and evaluation whilst also providing high quality care and support . The Care Coordinator will also have 2IC responsibility within the role.

To be successful, applicants will need to possess the following:

  • Current registration with AHPRA (RN)
  • Minimum of 5 years experience within an Aged Care setting
  • Well developed interpersonal skills within a team environment
  • Excellent written and verbal communication skills
  • Proven leadership skills and the ability to work within a multi-disciplinary team
  • Current evidence of clinical competence and continued education
